Documents for Minors

The rules differ slightly based on age (0-4 years vs above 4 years).

Essentials

  • ✅ Birth Certificate (Proof of DOB)
  • ✅ Present Address Proof (Can be parent's Aadhaar/Passport)
  • Annexure D (Declaration by Parents)

Parent's Documents

  • ✅ Valid Passport of both parents (Original + Copy)
  • ✅ OR Aadhaar/Voter ID if parents don't have passports

Validity & Fees Explained

0 - 15 Years 5 Years (or till 18) ₹1,000
15 - 18 Years 10 Years (Adult Validity) ₹1,500

Single Parent / Separated Parents

If parents are separated or one parent is not giving consent, special annexures are needed.

  • Annexure C: Used when one parent's consent is not available (divorce pending, single mother/father).
  • Divorce Decree: Court order awarding custody is helpful but not always mandatory if Annexure C is justified.

What is Annexure D?

It is a standard declaration signed by BOTH parents confirming they have no objection to the child getting a passport. It replaces the old need for sworn affidavits.

Important: Both parents should ideally be present at the PSK. If one cannot come, their signed Annexure D + self-attested passport copy is a must.
