The rules differ slightly based on age (0-4 years vs above 4 years).
Essentials

Parent's Documents
- ✅ Valid Passport of both parents (Original + Copy)
- ✅ OR Aadhaar/Voter ID if parents don't have passports
Validity & Fees Explained

Single Parent / Separated Parents
If parents are separated or one parent is not giving consent, special annexures are needed.
- Annexure C: Used when one parent's consent is not available (divorce pending, single mother/father).
- Divorce Decree: Court order awarding custody is helpful but not always mandatory if Annexure C is justified.
What is Annexure D?
It is a standard declaration signed by BOTH parents confirming they have no objection to the child getting a passport. It replaces the old need for sworn affidavits.
Important: Both parents should ideally be present at the PSK. If one cannot
come, their signed Annexure D + self-attested passport copy is a must.
